To all Isko/Iska aspirants, please be reminded that the UPCAT 2026 application deadline is on March 31, 2025. Make sure to complete all requirements on time to secure your application. You can access the online portal at https://upcat2026online.up.edu.ph/.
📌 List of Requirements:
✔ Form 1 (Personal Data Form) – To be filled out from March 3 to March 31, 2025. It is available on the UPCAT 2026 portal.
✔ 2×2 Picture with Name Tag and Signature – To be filled out from March 3 to March 31, 2025. The picture may be digitally edited, or the name and signature taken from a camera.
✔ Parent/Guardian Certification – To be filled out from March 3 to March 31, 2025. Upload a copy of your parent or guardian’s signature over printed name.
✔ Form 2A (Confirmation of Grade 11 Schools) – To be filled out from March 3 to April 7, 2025. This is done by your Grade 11 school. Kindly coordinate with them to have your application confirmed.
✔ Form 2B (Manual Encoding of Grades) – To be filled out from August 4 to August 25, 2025. You will be asked to encode your grades from Grade 8 to 11. You may use your report cards or soft copy of grades for reference.
✔ Certified True Copy of Form 137 (Grades 8-11) – To be personally submitted at the Office of Admissions, Diliman Campus from August 4 to August 25, 2025. Only certified true copies will be allowed, NOT original copies.
